About the role

The physical therapist will evaluate and treat patients in a home care setting to improve mobility and manage pain. They are also responsible for documenting patient progress and coordinating care with physicians and the broader healthcare team.

Benchmark Therapy and Rehab is a large therapy practice providing services to a multitude of providers in Home Health, and we are seeking compassionate and driven Physical Therapists to join our therapy team.

Position Summary

Physical Therapist- Home Care

Requirements: and Responsibilities:

Evaluates and provides initial treatment with patients recovering from injury, disease, or surgery in a home care environment.

Establishes treatment plans using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility.

Works cooperatively with physicians, case managers and treating therapists.

Monitors, documents, and reports patients' progress and communicates needed health information regarding the patient to all appropriate health care team members.

Requires state license to practice physical therapy.

Reports to manager of clinical services.
