Position Overview The Social Worker position plays a vital role in ensuring safe, timely, and effective transitions of care for patients. This position supports patients and families by coordinating post-hospital services, addressing social and financial needs, and promoting continuity of care.

Education: High School Diploma or equivalent – required Preferred Education: Bachelor’s degree in Social Work or related field Experience: Healthcare, case management, or social services experience preferred Core Competencies Strong communication and interpersonal skills Organizational and time management abilities Critical thinking and problem-solving skills Compassionate, patient-centered approach Ability to manage multiple priorities in a fast-paced environment Employment Requirements Compliance with all hospital policies, including vaccination requirements (approved exemptions considered) Successful completion of a criminal background check and pre-employment drug screening (post-offer)
