The Care Coordinator is responsible for completing CANS/ANSA assessments and coordinating behavioral health services for children and families. They manage client cases, connect individuals to community resources, and ensure all documentation meets Medicaid and agency standards.
Candidates must be bilingual in English and Spanish and meet Texas QMHP qualifications with a minimum of a Bachelor's degree in a human services field. Previous experience in case management and completing CANS/ANSA assessments is required.
Complete CANS/ANSA assessments accurately and timely Coordinate care and connect clients to community resources Maintain communication with families, providers, and referral sources Assist with service planning and treatment coordination Ensure documentation meets Medicaid, Texas, and agency standards Monitor client progress and identify service needs Maintain confidentiality and HIPAA compliance Support office operations and intake workflow as needed
