The Social Worker performs psychosocial assessments of patients and families to develop and implement effective care interventions. They also provide psychoeducation and serve as a consultant to the interdisciplinary group.
The role requires a professional committed to hospice care and interdisciplinary teamwork. Candidates must be able to provide supportive counseling and psychosocial interventions in an end-of-life care setting.
