The facilitator provides one-on-one support, supervision, and life skills training to individuals with developmental disabilities in various settings. They are responsible for assisting clients with community integration, transportation, and implementing person-centered plans.
Candidates must be at least 21 years old with a high school diploma or GED and a valid driver's license. Applicants must be able to perform physical tasks such as lifting up to 60 pounds and standing for the majority of the shift.
The incumbent is responsible for providing positive support and supervision to persons with developmental disabilities; to help them achieve their highest level of independence and quality of life in the home, community, and/or work setting.
This position is part-time working 10 - 15 hrs. per week based on the needs of the client.
$18.00 per hour
Requirements: Essential
Includes the following and or other duties as assigned to meet the vision, fulfill the mission and abide by the values of WCDC. Complies with all agency policies and procedures. Provides one on one assistance, support, training, companionship and supervision to individuals with disabilities who live in the community. Assists individuals in accessing community events, social outings, work sites, restaurants, friends, and entertainment. Provides life skill training, i.e. - activities of daily living, household tasks, time management, money management, shopping, social skills, etc. as directed by the Person Centered Plan (PCP). Safely transports individual(s) to events, etc. in a company or personal vehicle. Serves as a member of the PCP team and provide input for the annual meeting for assigned individual(s). Learns about assigned individual(s) disability and preferences; remain sensitive to individual(s) needs; counsel them to help them adjust to their disability; and enable them to make informed choices. Maintains formal and informal communications with immediate supervisor, keeping him/her aware of activities and concerns with the program; follow chain of command. Plans activities with the individual(s) based on PCP as well as expressed interests of the individual(s) Treats the individual(s) with dignity and respect at all times and act in a professional manner consistent with WCDC mission and vision. Develops strategies to help clients overcome their limitations and individualized strategies to provide for the needs of each individual served. Understands and follows all safety guidelines in accordance with each individual(s) PCP, including but not limited to care plans, behavior plans, and chocking protocols etc. Maintains positive communication with the family/guardian of assigned individual(s); act in a professional manner in accordance to WCDC mission and vision. Maintains regular and consistent adherence to assigned work schedule. Obtains/maintains all certifications and training as required by WCDC. Performs other duties as assigned.
Must be at least 21 years old, with a valid driver's license, and approved to transport clients. License may not be Provisionals and have a driving record that reflects responsibility and maturity. Complete all DDA Core Trainings successfully.
A High School Diploma/GED required.
Must be able to stand and walk 90% of work time; sit and/or drive 10%. Must be able to lift, carry, push, and/or pull at least 60 pounds. Must be able to stoop, kneel, crouch, and reach. All of the requirements are necessary in order to properly supervise and assist individuals with disabilities. Should be able to climb stairs/ladders and have good balance.
