The Resident Assessment Nurse conducts the Resident Assessment Instrument (RAI) process to identify resident needs and develop comprehensive care plans. They also coordinate with the interdisciplinary team to ensure accurate clinical documentation and participate in daily clinical huddles.
Candidates must be a Registered Nurse or an experienced LPN with at least one year of experience in a skilled nursing or long-term care facility. Proficiency in computer systems like Point Click Care and strong communication skills are essential for this role.
Description The Resident Assessment Nurse is responsible for conducting the Resident Assessment Instrument (RAI) process to ensure individual resident medical, psychosocial, emotional and physical needs are identified, and care plans are developed to assist residents in achieving their highest practicable level of function. The RAN is part of the facility team. The Administrator expects the RAN to be a visible, engaged member of the building, attending standing meetings, contributing to IDT discussions, and meeting building expectations on conduct and professionalism. Qualifications and Skills Registered Nurse (strongly preferred) or experienced LPN with a minimum one-year nursing experience in a skilled nursing/long-term care facility. Prefer Resident Assessment Coordinator (RAC) Certification. Excellent verbal and written communication skills with residents, families, all levels of staff and external customers. Proficient computer skills essential including the ability to use Point Click Care and conduct online submissions.? Must possess the ability to work harmoniously with and supervise other personnel. Must possess a high degree of personal accountability, responsibility and the ability to make independent decisions when circumstances warrant such action. Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures, etc., that are necessary for providing comprehensive activities to residents.
