Job Summary As a RN Case Manager at Ancora, you will play a crucial role in delivering exceptional home health and end-of-life care to our patients.
Your compassionate approach will ensure comprehensive physical, social, and psychological assessments and your friendly and supportive demeanor will actively engage patients, families, and the rest of our Interdisciplinary Group in developing comprehensive care strategies and plans.
With your attention to detail, you will efficiently coordinate patient care delivery, manage resources, and maintain meticulous patient records.
Your positive collaboration with other team members, patients, and their families, and proactive initiation of referrals will create a holistic care experience.

Position Requirements Schedule: This role operates on a 7 days on, 7 days off rotation throughout the year.
Minimum Commitment: Must work a minimum of 15 days per month, including 2 weekends.
While on call, you will be expected to handle all patient needs, including: Medically necessary scheduled visits. PRN (as-needed) visits. New patient admissions. Responding to and managing death calls. This structure ensures comprehensive care delivery and continuity for our patients during your on-call periods. Desired Skills & Qualifications Current Registered Nurse (RN) license in the state of practice Home Health / Hospice experience as a Registered Nurse preferred Ability to utilize full range of movement to transfer patients Effective verbal & written communication & time management skills Valid Driver's License (clean record) & reliable transportation.
