Supports and participates in care plan implementation by following assigned interventions related to skin integrity and nutrition.
Obtains and documents resident weights, assistance provided, resident responses, observations, etc., as scheduled and ordered using calibrated equipment and approved procedures
Ensures weights are obtained consistently such as at the same time of day, on the same scale, under same conditions when applicable, etc., in order to support clinical accuracy
Promptly reports significant or unexpected weight changes to the Charge Nurse and/or Director of Nursing
Assists residents safely during weight checks, using proper transfer techniques and assistive devices
Assists the Wound Nurse with wound care procedures by preparing supplies, positioning residents, and providing comfort, reassurance, and emotional support during potentially uncomfortable or sensitive procedures
Observes for and promptly reports changes in skin integrity, wound appearance, drainage, odor, or signs of discomfort or infection

Supports infection prevention practices by following hand hygiene, PPE, and clean/dirty workflow protocols
Explains care activities in a calm, supportive manner to promote resident understanding and cooperation
Fosters trust through compassionate communication and consistent, respectful interactions with both residents and coworkers
Provides respectful assistance during wound care while maintaining resident privacy and dignity
Honors resident preferences and right to refuse care and promptly documents and reports refusals of care to the Charge Nurse and/or Director of Nursing
Performs other duties as assigned.
SKILLS &
Qualifications:
High school diploma or GED with associate’s degree preferred.
Valid Tennessee Certified Nursing Assistant license in good standing
At least 1 year of CNA experience in a long-term care environment
Working knowledge of CMS regulations, resident rights, skin integrity principles, weight monitoring practices, and infection control standards
Proficiency in Microsoft Office products such as Outlook, Excel, Word, etc., with additional experience in patient management software strongly preferred.
Strong attention to detail and observational skills
Demonstrated ability to work effectively and efficiently with minimal direct supervision
Ability to follow care plans closely and work collaboratively with both Charge Nurses and the interdisciplinary team
Exceptional communication, documentation, organizational and time management skills
