The Palliative Care Coordinator is responsible for processing referrals, scheduling patient visits, and coordinating care goals between providers. They also serve as a liaison for the medical team, manage documentation, and ensure compliance with company policies.
Candidates must hold an active RN or LPN license in the state of practice and possess at least three years of nursing experience. Experience in palliative, hospice, or home health care is preferred, and CHPN certification is considered an asset.
