MDS Coordinator (RN/LPN/LVN) – Skilled Nursing Facility

A growing Skilled Nursing Facility is seeking a detail-oriented MDS Coordinator to join the team. This role is responsible for coordinating and completing accurate and timely MDS assessments while ensuring compliance with all state and federal regulations.

Key Responsibilities

Complete and oversee MDS assessments and care plans Ensure accuracy of documentation and regulatory compliance Collaborate with interdisciplinary team on resident care Monitor case mix and support reimbursement optimization

Qualifications

Active RN or LVN / LPN license MDS experience preferred Strong attention to detail and organizational skills Knowledge of CMS guidelines and long-term care regulations What’s Offered:
