The MDS Coordinator is responsible for completing and overseeing accurate MDS assessments and care plans while ensuring regulatory compliance. They also collaborate with the interdisciplinary team to monitor case mix and optimize reimbursement.
Candidates must hold an active RN or LVN/LPN license in Oklahoma or be able to obtain one. Experience with MDS and knowledge of CMS guidelines and long-term care regulations are preferred.

MDS Coordinator (RN/LPN/LVN) – Skilled Nursing Facility
A growing Skilled Nursing Facility is seeking a detail-oriented MDS Coordinator to join the team. This role is responsible for coordinating and completing accurate and timely MDS assessments while ensuring compliance with all state and federal regulations.
