T&N Services and Solutions is a dedicated behavioral health outpatient clinic in Washington, DC, specializing in mental health and Substance Use Disorder (SUD) treatment. As a small but mission-driven organization, we are committed to providing high-quality, compassionate care while upholding the highest standards of state and federal compliance. We are seeking additional Recovery Coaches to join our growing team. As a coach you will assist clients and families in the rehabilitation and recovery process from the challenges of dependency.
SUMMARY Recovery Coaches assist clients in defining their goals for recovery and assist them in meeting those goals.
The Substance Use Model involves an integrated, multidisciplinary team approach to providing high-intensity rehabilitative services, supports, and interventions to assist clients in achieving their personal recovery goals.
Much of this work involves teaching clients the skills necessary to successfully manage their illness and live as independently as possible.
In addition, coaches link clients to services and professionals that can assist in the clients’ recovery (care coordination).
Forming trusting relationships that are therapeutic and supportive is the manner in which recovery coaches are able to be most helpful for clients.
As they accompany clients to appointments, visit clients in their homes, and meet with clients where they may socialize, worship, and work, recovery coaches are able to facilitate recovery and integration in all aspects of a client's life.
Who Should Apply? Individuals with experience working in home health or home care, with demonstrated familiarity in working within the community. Home health and home care aides, personal care aides, and community support workers are highly encouraged to apply.
ESSENTIAL DUTIES Provide ongoing assessment of clients’ needs and facilitate the development and updating of the Individualized Recovery Plan.
Obtain and manage a caseload, ensuring active engagement with participants on an ongoing basis.
Ensure each participant receives a minimum of 1.5 hours of support per week as a recovery coach.
Facilitate at least one group session per week.
Teach clients how to successfully manage their illness through the development of symptom recognition skills, symptom management skills, and coping skills.
Teach clients any skill that will facilitate independence and achievement of their recovery goals, including daily living skills, interpersonal skills, independent transportation skills, and anger management skills.
Assist and support clients in crisis situations.
Assist clients in applying for all benefits and entitlements that will provide for their needs, such as SSI, Medicaid, Section 8, energy assistance, and reduced fare metrocards.
Assist clients in developing natural support networks by locating supports and services in the community such as AA, self-help groups, and recreational centers.
Collaborate with the Housing Team to locate safe, sanitary, and affordable housing for clients that aligns with their level of care needs, and provide support during transitions between different types of housing.
Accompany clients to critical services when necessary, especially critical medical appointments.
Provide education, support, and consultation to clients’ families and support systems, as appropriate.
Participate in the team meetings.
Provide advocacy and support for clients as they navigate social, governmental, and community systems.

Pay structure
Fee-for-service with minimum weekly billable hour requirements.
Performance Expectations
Proactively manage a caseload by engaging with clients at least twice per week and tracking key recovery metrics. Ensure compliance with federal and district laws, licensing requirements, and scope of practice guidelines. Facilitate at least 1 group session a week
