The Targeted Case Manager assists eligible individuals in accessing necessary recovery supports, medical, social, educational, and other services. Duties involve assessing individual needs, coordinating service provision, and accurately documenting services and outcomes.
Candidates must possess a Bachelor’s degree in social work or a human services field, along with one year of post-graduation work experience or a Master's Degree. A current driver’s license, basic computer skills, and strong documentation and communication abilities are also mandatory.
