About the role

Position Snapshot

The Service Coordinator will support AIM’s Nursing Home Transitions and Diversion (NHTD) and Traumatic Brain Injury (TBI) team, which provides support and services to people with disabilities and the elderly who wish to continue living in their homes. This is a part-time position with growth potential for full-time.

Essential Functions

Comply with all aspects and responsibilities, including the NY State Department of Health (DOH) NHTD and TBI service coordination waiver services.

Home visiting and community education.

Develop, review, and submit service plans.

Maintain orderly recordkeeping with strict attention to detail and articulate time tracking.

Participate in the development of the programs.

Case management for consumers participating in services. Up to 25 individuals.

Candidate Requirements

Persons employed as Service Coordinators must be a/an

1. Licensed Master Social Worker (LMSW- licensed by the NYS Education Department);

2. Licensed Clinical Social Worker (LCSW- licensed by the NYS Education Department);

3. Individual with a Doctorate or Master of Social Work degree;

4. Individual with a Doctorate or Master of Psychology degree;

5. Licensed Physical Therapist (licensed by the NYS Education Department);

6. Registered Professional Nurse (licensed by the NYS Education Department);

7. Certified Teacher of Students with Disabilities (certified by the NYS Education Department);

8. Certified Rehabilitation Counselor (certified as a Certified Rehabilitation Counselor (CRC) by the Commission on Rehabilitation Counselor Certification);
