The Case Manager provides compassionate support and connects individuals with mental health or developmental conditions to essential community resources. Responsibilities include monitoring treatment progress, conducting home visits, and assisting clients with housing, food, and healthcare needs.
Candidates must hold a Bachelor's degree in a social service-related field such as Social Work or Psychology. Previous experience in a social services role is preferred, along with strong organizational and communication skills.
