The Physical Therapist is responsible for conducting one-on-one patient care and providing education to patients and their families in a home setting. They will assess patient needs, develop personalized treatment plans, and implement therapeutic interventions to improve functional abilities.
Candidates must be graduates of an accredited physical therapy program and hold a current state license. Additionally, they must possess current CPR certification, reliable transportation, and excellent communication skills.

You will work directly with patients in their homes, assessing their needs, developing personalized treatment plans, and implementing therapeutic interventions to optimize their functional abilities.
