Lincoln Knolls is seeking compassionate, motivated, and skilled therapy professionals to join our interdisciplinary team on a PRN (as-needed) basis. If you are passionate about helping residents regain independence and improve quality of life in a skilled nursing setting, we want to hear from you!

  • PRN Physical Therapist Assistant (PTA)
  • PRN Speech-Language Pathologist (SLP)

Position overview

Our therapy team plays a critical role in delivering high-quality rehabilitative care to our residents. PRN team members will provide coverage as needed, supporting evaluations, treatments, and care plan development in collaboration with physicians, nursing, and other disciplines.

  • Key responsibilities
  • Provide evidence-based therapeutic interventions tailored to resident needs
  • Complete evaluations, treatment plans, and progress notes in accordance with regulatory and facility guidelines
  • Collaborate with interdisciplinary team members to ensure optimal outcomes
  • Educate residents and families on treatment plans and goals
  • Maintain compliance with Medicare, Medicaid, and skilled nursing facility regulations
  • Accurately document services in a timely manner

Qualifications

For OTR

  • Graduate of an accredited Occupational Therapy program
  • Current Ohio Occupational Therapy License (issued by the Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board)
  • Current NBCOT certification preferred

For PTA

  • Graduate of an accredited Physical Therapist Assistant program
  • Current Ohio PTA License
  • Must work under the supervision of a licensed Physical Therapist

For SLP

  • Graduate of an accredited Speech-Language Pathology program
  • Current Ohio SLP License (issued by the State of Ohio Speech and Hearing Professionals Board)
  • Certificate of Clinical Competence (CCC-SLP) preferred

All Positions

  • Current CPR/BLS certification
  • Experience in a Skilled Nursing Facility (SNF) or post-acute setting preferred
  • Strong clinical, communication, and documentation skills
  • Knowledge of Medicare Part A & B guidelines is a plus

Requirements
