• PM, with one scheduled evening per week extending to 6:30 PM

Job Title: Licensed Practical Nurse (LPN) Primary Care

Reports to: Primary Care Nurse Supervisor

Areas of Responsibility

Function as LPN in the provision of health care and educational/screening programs.

Major Duties

• 1.
• Work within the LPN’s Scope of Practice performing skills including, but not limited to the following: 1.
• Patient assessment and monitoring, including preventative screenings 2.
• Prepare patients for examinations and explaining procedures 3.
• Administer medication as ordered 4.
• Administering immunizations 5.
• Assist with minor procedures 6.
• Wound care and dressing change 7.
• Data collection and reporting 8.
• Routine lad and sample collection including phlebotomy 9.
• Triage and assist in emergency situations as needed, according to the provider’s written, verbal, or electronic orders 10.
• Provide emotional support and counseling to patients, caregivers and families as needed 2.
• Assist with the external referral process, assist with medication refills, and respond to patient inquiries with timely follow-up.
• 3.
• Assist with coordinating patient appointments and follow up care as needed.
• 4.
• Care-manage chronic diseases in patients by identifying barriers to care and offering information related to specific diseases and initiating referrals for needed in-house services (Pharmacist, Diabetes Educator, Dietician, Social Worker etc.) to improve patient outcomes.
• 5.
• Act as a patient advocate and practice effective patient communication.
• 6.
• Assist patients to complete appropriate applications for eligibility of patient assistance medication programs; coordinates and monitors the need for reordering medications from patient assistance medication programs.
• 7.
• Provide Health Education to patients and families about their conditions, medications, at-home self-care plans and health promotion activities.
• 8.
• Work cooperatively with all CAPWN Health Center staff, other agencies and programs, and with all program staff in meeting agency/program goals; Become knowledgeable of community and CAPWN resources.
• 9.
• Ensure clinical areas and equipment are clean and sterile.
• 10.
• Ensure compliance with healthcare regulations and standards 11.
• Assist in the management of medical supplies and inventory 12.
• Practice cultural competence with the culturally diverse population this clinic serves.
• 13.
• Effectively communicates while interacting with patients, families and staff and recognizing the complexity in language interpretation.
• 14.
• Maintain a positive and professional attitude, striving to openly communicate with co-workers and patients, breaking down communication barriers when necessary.
• 15.
• Participate in Quality Assurance and Improvement initiatives to ensure the highest standards of patient care are met.
• 16.
• Maintain confidentiality of records and communications following CAPWN policy and HIPAA privacy and security policies and procedures.
• 17.
• Complete all documentation thoroughly and in a timely manner to ensure patient records are up-to-date and accurate.
• 18.
• Utilize the Electronic Medical Record (EMR) to assist in the delivery of quality care and track patient care.
• 19.
• Attend staff meetings, training, and workshops or classes as requested/required by Program Managers, Directors.
• 20.
• Maintain current BLS (Basic Life Support) 21.
• Maintain continuing education requirements.
• 22.

Working Conditions

• Clinic environment, training, meetings, and community interaction.
• Well-lit, temperature-controlled clinical setting, with standard medical and office equipment available.
• Work may require meeting tight deadlines.
• Work within a program in a state of change with multiple interruptions and interactions.
• May work irregular hours and attend out of town training.
• The noise level in the work environment is average.

• Physical Requirements
• Ability to frequently bend, stoop, twist, stand and walk.
• Manual dexterity sufficient to operate office and medical equipment.
• Must regularly talk, hear and have close vision ability to record and communicate appropriate documents and records.
• Occasionally lifting and carrying up to 25 pounds.
• Physically able to reposition patients and lift arms above shoulder level.
• Physically able to push a patient in a wheelchair.

Minimum qualifications

GRADE: 12

1. Current Nebraska license as an LPN. 2. Current CPR certification. 3. Strong interpersonal skills and proficient level of oral and written communication, 4. Promote teamwork and ability to motivate staff 5. Advanced level of organization and multi-tasking abilities.

Grade: 13

1. Meet the minimum qualifications above; 2. Three years of nursing experience preferably in clinical nursing, women’s health, public health, or community health nursing.

Grade: 14

1. Meet the minimum qualifications above; 2. Six years of nursing experience preferably in clinical nursing, women’s health, public health, or community health nursing.
