The Social Worker will conduct psychosocial assessments and provide emotional support, crisis intervention, and resource coordination for hospice patients and their families. They will also collaborate with the interdisciplinary team to develop and implement individualized plans of care that ensure patient comfort and dignity.
Candidates must hold a Master’s degree in Social Work (MSW) and possess strong communication skills. One year of hospice social work experience is preferred, and the role requires a valid driver's license and reliable transportation for home visits.

Master’s degree in Social Work (MSW). One year of social work experience in a hospice setting preferred; strong understanding of hospice philosophy and end-of-life issues. Ability to work tactfully with patients, families, and the community. If performing home visits: must be a licensed driver with an insured, reliable automobile.
