Provide personalized one-on-one care to patients in their homes, including assistance with activities of daily living and mobility. Maintain accurate records of patient conditions and report observations to supervisors.
Must be a certified Home Health Aide or CNA with experience in dementia, aphasia, and ambulatory care. A valid driver's license and access to a car are required for this position.
Must have experience working with people with Dementia/Aphasia and Ambulatory: Maintain records of patient care, conditions, progress or problems to report and discuss observations with supervisor. TOTAL CARE Assist client with full ADLs (Activities of daily living) Assist with cooking, medication reminders, extensive transfer support (bed, chair, mobility transitions) Assist with ambulation (wheelchair support) Skilled services (wound care) Hoyer Lift exp.
