• Responsibilities: Complete evaluations assessing residents’ functional, cognitive, and physical abilities Develop and implement individualized, goal-driven treatment plans Provide OT interventions focused on ADLs, self-care, upper extremity strength, and functional mobility Educate residents, families, and staff on therapy goals and strategies Collaborate with interdisciplinary team members to ensure comprehensive care Maintain timely and accurate documentation per regulatory standards Promote a safe, encouraging therapeutic environment that supports resident independence
