The Physical Therapist will evaluate residents and create individualized treatment plans to support their mobility and independence. Collaboration with interdisciplinary teams for resident-centered care is also a key responsibility.
Candidates must have an active Wisconsin Physical Therapist license and strong clinical, communication, and documentation skills. A passion for working with older adults in assisted living is essential.
