Manages patient care by performing psychosocial assessments and coordinating discharge plans to ensure continuity of care. Collaborates with multidisciplinary teams to provide supportive counseling and crisis intervention for patients and families.
Requires a Master of Social Work and an LMSW license, with a preference for LCSW. Minimum of 2 years of clinical experience in a medical or clinical setting is required.
Manages the care for a specific group of patients; facilitates the safe movement of patients through the continuum of care, ensures optimal utilization of resources, service delivery and compliance with external review agencies, local, state, and federal requirements. Service includes psychosocial assessments, developing and coordinating discharge plans to insure continuity of care, providing supportive counseling, and crisis intervention to enhance coping and functioning ability. Performs other duties as assigned.
