The Social Worker provides psychosocial assessments and support services to hospice patients and their families to help them cope with illness and death. Responsibilities include developing care plans, facilitating bereavement groups, and coordinating with an interdisciplinary team.
A Master's degree in Social Work from an accredited school is required. Candidates must have at least one year of healthcare experience and a valid Tennessee driver's license with insurance.

EDUCATION and/or EXPERIENCE Master's degree in Social Work from an accredited school of social work. One (1) year of work experience in a healthcare setting required. Hospice experience preferred.
CERTIFICATES, LICENSES, REGISTRATIONS Current and valid Tennessee driver’s license and automobile insurance as required by Tennessee State Law.
