Deliver physical therapy care to adult and geriatric patients while collaborating with a clinical team. Responsibilities include preparing documentation, progress notes, and establishing patient plans of care.
Candidates must possess a valid State Physical Therapy Assistant License and credentials. Experience in Skilled Nursing Facilities (SNF) or acute care is required.
We are hiring a Physical Therapy Assistant to deliver care to our patients and work with a diverse team of clinical professionals.
Provide physical therapy to adult and geriatric patients Prepare documentation, progress notes, and discharge summaries Establish a plan of care in accordance with patients' needs and capabilities Travel to nearby centers as needed
