Job description

RESPONSIBILITIES\n\n * Conducting pre-enrollment assessments for prospective enrollees utilizing\n clinical nursing expertise and critical thinking skills including obtaining\n preventive health service data. \n * Completes UAS - New York Assessment instrument to determine continuing\n eligibility and / or identify any needed changes to Care Plan.\n * Utilizes a skillful approach for assessing, coordinating, and evaluating\n consumer needs and will need to develop a prospective plan of care. \n * Communicates with the Care Team and participates in interdisciplinary team\n meetings\n * Responsible for environmental assessments.\n * Completes semi-annual assessments as requested\n * Maintains flexibility in schedule and responds to unexpected emergencies and\n changes in workload to fulfill responsibilities. \n * Conducts chart audits as requested if time permitting\n\n\nQUALIFICATIONS\n\n * Current NY State Registered Nurse License \n\n \n\nEducation: \n\n * Bachelor’s Degree in Nursing
