The Counselor II provides assessment, treatment planning, and direct therapeutic interventions for patients and families in addiction treatment. Responsibilities include conducting group sessions, managing admissions/discharges, and coordinating care with medical teams and outside providers.
Requires a high school diploma and at least two years of experience in addiction or behavioral health. Candidates must hold a valid addiction counselor certification (CATC, CADC, or LAADC) and BLS-AHA certification.
The Counselor II supports the SolMar Retreat and Hoag Addiction Treatment Center (HATC) treatment team in the assessment, treatment planning, referral recommendations, discharge planning, education and direct therapeutic intervention for patient and their families throughout the course of treatment. Supports the goals of the patient’s treatment plan and assists in the clinical care and case management. Coordinates care with outside providers by referrals when necessary. Performs other duties as assigned
In addition to the above, performs focused collaboration with the medical team, admissions and the utilization review team.
In addition to the above, Counselor II assists with admissions and discharges, assuring all admissions and discharge documents are signed. Conducts Screenings/ intake assessments as needed and within the incidental medical services (IMS) guidelines found in the IMS policies and procedures manual. Provides client with an orientation about the program, rules, and a tour of the unit, including: their room as assigned. Assist on searching belongings at the time of admission as well as random room searches in residential level of care. Intervenes as an advocate for clients or patients to resolve emergency problems in crisis situations.
Documents daily living activities and vital information. Records patient behaviors and inform Supervisor of Addiction Care and Clinical Manager of any changes in behavior. Receives and presents change of shift reports as assigned.
