About the role

The Community Health Worker supports the Pediatrician and Population Health teams by recruiting and supporting patients to engage in care management services. They provide patient education, advocate for access to services, and coordinate referrals to internal and external providers.

This is an onsite position where the Community Health Worker will be supporting our Pediatrician Team.

Position Summary

The Community Health Worker/Patient Navigator (CHW) will work to outreach, identify, recruit, and support patients to engage in care and/or care management services; and will provide patient education and advocacy to assist patients with accessing services of FHCW and make and follow-up on referrals for external services as part of the population health team. The Community Health Worker will support the Care Management Team and the Population Health Team.

Program Specific Essential Duties and Responsibilities

Supports the care team with patient follow-up, reminders, and addresses barriers to care.

Document all services as required and will assist in meeting all program requirements.

Collects and analyzes data related to the project and works with data management staff for data collection.

Provides outreach and meets with patients to explain services and coordinates the treatment plan.

Facilitates referral process for patients using Healthify, E-referral, Community Help, and other tools as applicable.

Advocates for patients regarding transportation, interpretation, health insurance, and FHCW services.

Strong organizational and communication skills to act as a hub of information across multiple departments.

Any and all other duties as assigned.

High school diploma or GED required; Bachelor’s degree is preferred. Preferred candidate will have Community Health Worker or Medical Interpreter certificate of completion.

  • Minimum of two years relevant work or college-level internship experience required. Proven ability required to perform patient interviews, coordinate and collaborate with internal services and external community partners.
  • Bilingual in English and another language appropriate to FHCW patient population required.
  • Community Health Worker Core Training required within first year of employment.

Reporting Relationship: Director of Population Health

Monday-Friday; 8: 30am -5:00pm

Requires a high school diploma or GED, with a bachelor's degree and CHW or Medical Interpreter certification preferred. Candidates must have at least two years of relevant experience and be bilingual in English and another language.
