$90,000 - $120,000 / YEAR
Provide primary care services, including physical exams and diagnosis of acute and chronic conditions, for residents in skilled nursing and long-term care facilities. Collaborate with interdisciplinary teams to develop individualized care plans and educate patients and families on treatment goals.
Requires an active and unrestricted NP, PA, or Physician license in Minnesota. Candidates must have a minimum of 6 months of experience in long-term care or skilled nursing facilities.

The Company is currently hiring a Primary Care Nurse Practitioner/Physician’s Assistant (NP/PA) to provide on-site care to residents in long-term care settings. This is a part-time position ideal for a provider who values meaningful patient relationships and a collaborative care environment.
