Our Care Teams include a Physician, Nurse Practitioner, Care Coordinator, Medical Assistant, and support from a Nurse Care Manager. Nurse Practitioners are integral members of our Care Teams as they provide longitudinal care and build meaningful patient relationships; this creates trust and helps our patients understand and feel connected to their care plan.
Together this team is responsible for providing and coordinating care for an intimate panel of patients in our neighborhoods. As a key member of Sage Health care team, you will perform annual health assessments, complete post discharge visits and deliver cost-effective, quality care to assigned patients both in our centers and patients' homes.
Must hold a current NP or PA license, DEA certificate, and BLS certification. Requires at least two years of experience as an NP/PA, specifically in outpatient primary care or adult/geriatrics.
