Forest Hills Queens Primary Care Center is looking for a dedicated Primary Care Physician to join our healthcare team in Forest Hills Queens.

The successful candidate will play a vital role in delivering high-quality, patient-centered primary care services.

This position involves diagnosing and managing a broad spectrum of health conditions, emphasizing preventive medicine and chronic disease management.

You will collaborate with a skilled team of healthcare professionals to ensure comprehensive and coordinated care, promoting overall well-being for patients of all ages.

Responsibilities

Provide primary care services including diagnosis, treatment, and management of acute and chronic diseases. Conduct preventive health screenings and routine check-ups tailored to patients' needs. Establish and maintain ongoing relationships with patients to support health maintenance. Utilize electronic health records to document patient information accurately. Coordinate care with specialists and other healthcare providers to optimize patient outcomes. Educate patients on lifestyle modifications and disease prevention strategies. Participate in quality improvement initiatives and adhere to established clinical guidelines. Medical degree (MD or DO) from an accredited institution. Board certification or eligibility in Family Medicine, Internal Medicine, or related specialty. Valid medical license to practice primary care medicine. Experience in outpatient primary care settings is preferred. Strong clinical decision-making and interpersonal skills. Ability to work effectively within a multidisciplinary team environment. Commitment to patient-centered care and ongoing professional learning.

Requirements

Candidates must hold an MD or DO degree and be board certified or eligible in Family or Internal Medicine. A valid medical license is required, and experience in outpatient primary care is preferred.
