Responsible for helping patients maintain, restore, and improve movement and function within a home health setting. Duties include providing interventions, conducting regular reexaminations, and managing discharge planning.
Requires a current state license to practice physical therapy and graduation from a CAPTE-accredited program. Candidates should have one year of related experience or approval from clinical leaders, along with current CPR certification.
Responsible for helping patients maintain, restore and improve movement, activity and function. Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning. Other duties as assigned.
