The Care Management Assistant provides discharge planning support to Care Managers and Social Workers. They focus on reducing non-clinical barriers to discharge for patients and families in Inpatient and Emergency Department environments.
A high school diploma or GED is required, while an associate's degree in human services or a health-related field is preferred. Candidates must have at least 2 years of experience in a healthcare or social services setting.
30- 5 or 8-4:30pm if available. Training MUST be Monday - Friday for the first few weeks
As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families / caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments.
Education H.S.
