Perform comprehensive patient assessments to establish individualized care plans and facilitate communication with the interdisciplinary team. Provide patient education to promote self-advocacy and independence while documenting services using state-of-the-art technology.
Requires a valid Connecticut RN license and at least one year of RN experience, with homecare experience preferred. A valid driver's license, reliable transportation, and computer proficiency are mandatory.
