Provide remote and direct clinical support to individuals with disabilities to promote independent, self-directed living. Responsibilities include designing individualized care plans, managing housing stability, and coordinating with community partners.

Credentialed: Obtain and maintain SHIFT credentialing as ETIS. Associate degree in related field, Bachelors preferred. Experience supporting people with disabilities and/ or Mental Health Issues.

SJCS employees must: Obtain a valid health certificate to meet current funding authority requirements as required. Having a valid state-issued driver’s license with a current driving record in good standing is required. Vehicle Insurance Meet SJCS and the funding authority requirement of criminal background and record checks.
Physical and environmental conditions include: Work requires no unusual demand for physical effort except the normal risks or discomforts, requiring special safety precautions for operating a motor vehicle. Exposure to aggressive human behavior. Ability to lift up to 30 pounds. Travel up to 30%. Travel to Martin and Jackson, TN 3 to 4 times per year The hiring range for this position is $17.50-$18.25 per diam
