Provide one-to-one patient care including assistance with activities of daily living, medication reminders, and symptom monitoring. Maintain a sanitized home environment and document patient progress using a digital charting system.
Requires completion of a state-approved Home Health Aide program, CNA, or similar training with at least 2 years of relevant experience. Must possess a valid driver's license, reliable transportation, and CPR certification.

Key Responsibilities Hands-On ADL Support – Safely assist transfers, bathing, grooming, toileting, and mobility—always protecting dignity.
Home Environment & Infection Control – Change linens, do light laundry, and maintain a hazard-free, sanitized space.
Medication & Symptom Monitoring – Provide reminders, observe for changes, and document vitals or red-flag signs for your supervising nurse.
Therapeutic Engagement – Read aloud, reminisce, guide range-of-motion exercises, or accompany short walks to boost mental and physical wellness.
Accurate Documentation – Record care notes, progress, and concerns in our digital charting system before each shift ends.
What You’ll Need to Succeed Completion of a state-approved Home Health Aide program, CNA, or similar trainings
Minimum 2 years of relevant experience.
Valid driver’s license and reliable transportation to reach clients’ homes.
CPR certification (or willingness to obtain within 30 days).
Clear, confident communication and a passion for relationship-centered care.
