The Home Health RN is responsible for planning, coordinating, and documenting care for an assigned caseload of patients in their homes. This includes performing initial evaluations, implementing care plans, and collaborating with physicians and case managers to ensure regulatory compliance.
Responsible for planning, coordinating, and documenting care for an assigned caseload of patients. Utilizing the physician's orders and professional skills, the Home Health RN, in conjunction with the Case Manager, develops and implements a home health plan that meets each patient's specific needs and is in compliance with accrediting organization standards, Federal and State regulations, reimbursement guidelines, as well as agency policies and procedures. Participates in Quality Assessment and Performance Improvement (QAPI) activities.
