Provide comprehensive longitudinal care and late effects monitoring for pediatric and adult bone marrow transplant survivors. Perform medical evaluations for healthy related stem cell donors and coordinate multidisciplinary care plans.
Requires a Master's or Doctoral degree as a Nurse Practitioner, Physician Assistant, or Clinical Nurse Specialist. Must possess valid state licensure, DEA/CDS certifications, and BLS certification.
The Bone Marrow Transplant Survivorship Advanced Practice Provider provides comprehensive, longitudinal care to pediatric and adult patients following hematopoietic stem cell transplantation, with a particular focus on survivors of pediatric transplant transitioning into adulthood. This role emphasizes late effects monitoring, chronic condition management, survivorship care planning, and coordination of multidisciplinary care. The APP also performs medical evaluations of healthy related stem cell donors, both pediatric and adult, in close collaboration with transplant program leadership, coordinators, and primary care providers.
Minimum Education Master's Degree Nurse Practitioner Master's or Doctoral degree as a Nurse Practitioner from an accredited program and eligibility for medical staff credentialing required.
Physician Assistant Master’s or Doctoral degree of Physician Assistant Studies from an accredited program and eligibility for medical staff credentialing required.
Clinical Nurse Specialist Master’s or Doctorate degree in the clinical specialty area in which they practice from an accredited program and eligibility for medical staff credentialing required. (Required)
