About the role

Serve as the primary support and case manager for individuals with mental illness and substance abuse challenges. Assist clients in living independently within their community through personalized care planning and wraparound support.

Your official job title at Telecare will be Case Manager Substance Use Counselor Certified Fully In-Person Position, no work from home, no hybrid What You Will Do to Change Lives The Case Manager Substance Use Counselor Certified serves as the primary support and case manager for individuals with mental illness by assisting them to live as independently as possible in their chosen community, focusing on substance abuse and addictions. Shifts Available:

Full Time; Shift Hour and Days vary as needed.

Schedules may vary based on program needs and individual preferences. Typical shifts are as follows:

Morning Shifts: Start between 8:00 AM – 9:00 AM and end between 5:00 PM – 6:00 PM, depending on start time and lunch break duration.

Afternoon Shifts: One required per month, generally 11:00 AM – 7:30 PM or 8:00 PM, depending on lunch break.

Weekends & Holidays: 9:00 AM – 5:30 PM.

Weekend Requirement: Staff are required to work one weekend shift per month.

  • Expected starting wage range is $26.46 - $32.69. The full wage range goes up to $38.93. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements. What You Bring to the Table (Must Have)
  • High School Diploma or G.E.D
  • Washington State Substance Use Disorder Professional (SUDP) credential.
  • Six (6) months’ experience in addictions setting
  • Personal vehicle: Willing to use for work and willing to transport clients in your vehicle
  • Personal vehicle insurance with your name listed as a driver
  • Current vehicle registration
  • Your vehicle must pass safety inspection, paid by Telecare. Will include items such as breaks, break pads, headlights, clear windows (no large cracks), etc.

Company mileage will be reimbursed at the current Federal Rate

PACT is an Assertive Community Treatment (ACT) program operated by Telecare Corporation in Washington State. This program provides intensive, community-based behavioral health services to adults living with serious mental illness, many of whom also experience co-occurring substance use challenges.

Grounded in the recovery model, PACT is designed to meet individuals where they are—both literally and clinically. Services are delivered in the community and tailored to each person’s unique goals, strengths, and stage of recovery. The multidisciplinary team offers comprehensive, wraparound support, including psychiatric care, medication management, case management, and skill-building interventions.

A key focus of the program is supporting individuals with co-occurring disorders. While PACT does not provide detoxification services, the team emphasizes rehabilitation, harm reduction, and long-term recovery. Services are centered on helping individuals build stability through meaningful social supports, access to safe and sustainable housing, and connection to community resources.

Through a person-centered approach, the program prioritizes individualized care planning, empowering members served to define and pursue their own recovery goals—whether that includes improving health, strengthening relationships, increasing independence, or achieving greater community integration.

Requirements

Must possess a High School Diploma or GED and a Washington State Substance Use Disorder Professional (SUDP) credential. Requires at least six months of experience in an addictions setting and a reliable personal vehicle for client transport.
