The Nurse Practitioner will conduct routine primary care and follow-up visits for patients in long-term care and subacute nursing facilities. Responsibilities include diagnosing and treating diseases, managing chronic conditions, and coordinating care with facility staff and supervising physicians.

Job Summary Job Summary: We are currently seeking motivated Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our team and serve multiple nursing home facilities across NYC.

This is an excellent opportunity for new graduates looking to start their careers in a supportive and patient-focused environment.

The Nurse Practitioner will be part of Essen growing medical professionals.

The NP/PA will conduct routine primary care and follow-up visits for patients residing in long-term care and subacute nursing facilities.

The role includes physical assessments, chronic disease management, coordination with facility staff, and collaboration with our supervising physicians and care team.

Responsibilities

Responsibilities include: Examines, diagnoses and treats patients Ensures fulfillment of Essen Health Care commitment for ensuring quality care Diagnoses and treats variety of diseases in general practice Collects, records and maintains accurate patient information Documents items such as: past medical, family, and social history, review of systems, examinations, medications, allergies, assessment and plan Orders or executes various tests, analyses, and diagnostic images to provide information on patient's condition Analyzes reports and findings of tests and of examination, and diagnoses condition Administers or prescribes treatments Orders lab and imaging tests to determine and manage an immediate treatment plan Vaccinates patients to immunize patients from communicable diseases Advises patients concerning diet, hygiene, and methods for prevention of disease Consult with appropriate medical specialist when necessary; refer patient to subspecialists within network Providing care to patients in the Nursing home Training Requirement for Recent Graduates & Candidates New to the Nursing Home Setting:Recent graduates and candidates without prior Nursing Home experience must be willing and able to complete a comprehensive training program at our New York locations. This hands-on training is required before transitioning to our Bergen County, New Jersey Nursing Home locations as they become fully operational. Candidates should be comfortable commuting to New York during the training period. #INDHP

Qualifications

Job Requirements: Master's Degree in a Nurse Practitioner Program: Family or Adult Geriatrics Current New York State License as a Nurse Practitioner. Requirements
