The Transfer Center Nurse facilitates safe patient transfers by triaging clinical data and coordinating bed placement. They act as the primary clinical liaison between external hospitals and the health system to ensure timely and appropriate transitions.
Candidates must be a graduate of an accredited nursing school with an active RN or LPN license and BLS certification. 2-5 years of acute care experience is required, with a preference for BSN, ACLS, and ER or ICU backgrounds.

Transfer Coordination: Process incoming requests for transfers, direct admissions, and consultations from various referral sources. Clinical Triage & Triage: Utilize clinical expertise to assess patient acuity and urgency, recommending the necessary level of care and mode of transportation (ground or air). Bed Management: Collaborate with house supervisors and nursing units to assign beds based on current facility capacity and service capabilities. Liaison & Communication: Facilitate physician-to-physician and nurse-to-nurse reports, acting as a "clinical air traffic controller" between sending and receiving teams. Regulatory Compliance: Ensure all transfers adhere to EMTALA, HIPAA, and other regulatory standards.
Accurately record all tasks, conversations, and medical information into electronic health records (EHR) and transfer tracking software.
