Requires a Master's degree from an accredited school of social work. Must obtain LMSW licensure within 1.5 years of hire or possess a valid NYS limited permit/license.
The GCH Pediatric Mobile Clinic Initiative Social Worker is part of an interdisciplinary team providing pediatric primary care services to patients in the community with the goal of connecting or re-connecting patients to a medical home. The Pediatric Social Worker is responsible for the provision of services to children and their families who receive care through the Pediatric Mobile Clinic. This includes psychosocial assessment of identified risks and needs, and supportive intervention(s) to enhance health outcomes and health care utilization. Strong written and verbal communication skills, flexibility, and comfort working in a changing clinical environment is necessary ESSENTIAL FUNCTIONS Assess patients identified in need of social work intervention. Provide social work interventions and supportive counseling to address identified risks and SDOH. Provide connections to behavioral health, health home, and other community- based services as indicated. Provide or link families with needed resources such as housing, transportation, pharmacy, food, essential items, day care, legal, insurance and DHS. Collaborate with schools, day care, behavioral health, child welfare, early intervention, home visiting programs and other community-based services as part of the care plan. Collaborate with members of the health care team around matters related to patient care and enhancing outcomes as well as with other social workers at GCH to ensure continuity of SWK care between settings. Identifies and intervene around risk management issues, communicating these issues as needed, with the Pediatric Social Work Director or designee. Manages all cases of suspected child abuse and neglect in collaboration with the health care team. Documents social work activity in the patient’s medical record utilizing the pediatric, psychosocial assessment and progress notes per SWD policy Participates in activities of the Social Work Division including but not limited to staff meetings, designating time spent in the patients’ medical record, following absence approval process, participating in professional development and completion of other Hospital or Division forms; Hospital and Division Mandatory in-service training programs, and UHS reviews. May serve on a Division Committee May participate in teaching of medical, nursing and social work students/staff Other duties as assigned MINIMUM EDUCATION & EXPERIENCE Master's degree from an accredited school of social work required Or equivalent combination of education and experience LICENSES AND CERTIFICATIONS LMSW - Licensed Medical Social Worker - State Licensure within 1-1/2 years required or If duties are performed under OMH a minimum of a limited permit (not to exceed 1 year) or NYS Licensure upon hire required.
