Coordinates and implements patient care services for hospice patients and their families. Focuses on providing emotional, spiritual, and physical palliative care.
Requires a current NYS Registered Nurse license and at least two years of nursing experience in medical-surgical, oncological, or palliative care. A valid driver's license and vehicle are required for territories outside of Manhattan.
Assists in the planning, implementation, documentation and coordination of patient care services to Hospice patients and their families.
Current NYS Registered Nurse license. Two (2) years experience in nursing which includes any combination of medical-surgical, oncological, palliative and community home health/hospice. At least one (1) year of community hospice or home health experience is preferred. Hospice experience preferred. Position requires ability to commute to multiple territories to ensure continuity of care. A valid driver’s license is required. (This requirement does not apply to positions assigned exclusively to Manhattan territory) A vehicle is required to commute to patients. (This requirement does not apply to positions assigned exclusively to Manhattan territory) Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.
Baccalaureate Degree in Nursing from an accredited school is preferred.
