The LPN is responsible for providing home-based patient care, including monitoring vital signs and performing specialized procedures like catheter changes. They must collaborate with RNs and physicians to implement care plans and educate patients and caregivers on self-care techniques.
Candidates must have at least one year of clinical LPN experience and a current state LPN license. Additional requirements include CPR certification, a valid driver's license, reliable transportation, and liability insurance.
