The Case Manager supports individuals with serious mental illness and substance use disorders transitioning from incarceration to the community. Key duties include developing individualized reentry plans and coordinating essential behavioral health and social services.
The candidate must be able to conduct comprehensive assessments and manage behavioral health crises. Proficiency in trauma-informed, person-centered care and cross-system collaboration is required.

The Stepping Up Case Manager provides support to individuals with serious mental illness (SMI), substance use disorders (SUD), or co-occurring conditions as they transition from incarceration back into the community. This position is part of the Stepping Up Initiative, aimed at reducing the number of individuals with behavioral health conditions in jails. The Case Manager will work with incarcerated individuals nearing the end of their sentence to provide reentry planning, support, and linkage to essential community services that promote recovery and long-term stability.
