The LNA provides direct patient care in homecare settings and communicates patient needs to clinical teams. Responsibilities include following care plans, documenting patient status, and adhering to infection control protocols.
Part-Time (24 - 31 hours per week)
_______________________________________________________________________________ SUMMARY/OBJECTIVE: The LNAs work directly with patients and communicate on an ongoing basis with the clinical teams in regards to the patients and their changing needs.
The LNA is accountable to the Clinical Manager.
1. Reasonable accommodations may be needed to enable individuals with disabilities to perform the essential functions. 2. Completes at least an hour of education each month per CMS regulations and tracks it on their competency sheet. 3. Functions within the limits of LNA role and complies with all state & regulatory rules and follows the patient care plan including personnel care, housekeeping, errands, etc. 4. Informs the primary clinician / Case Manager of changes in patient condition, as appropriate and documents interdisciplinary communications. 5. Ensures that clinical competencies are completed prior to providing standard and specialized care and that competency sheet and/or patient record demonstrates. 6. Documents patient care per agency protocol and CMS regulations. 7. Collaborates with the interdisciplinary team 8. Adheres to Agency handwashing technique, infection control and bag technique. 9. Is an active participant in Case Management and agency in-services.

Supervisory responsibility: NONE
The job operates in field, entering into patient homes and locations in the community. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This position also travels in their own vehicle.
This is largely a traveling role; however, some sedentary action is required. This will require the ability to lift files, open filing cabinets and bend or stand as necessary.
Travel is expected for this position.
Must be at least 18 years old and must have completed an approved License Nurse’s Assistant Training Program. Six months of acute care experience is preferred.
Current N.H. State Registration as a Licensed Nurses Assistant (LNA) and in good standing CPR Certified within 6 months of hire Must be able to read, write and speak English fluently Must be available to work Friday, Saturday, Sunday and Monday if needed Must be willing to provide weekend services and to cover call. Must be able to supply a valid driver’s license without restrictions Must be able to sit for long periods of time viewing small print Must be able to comply with PBCH safety and dress code policies Must be able to meet the lifting requirements of the position Must pass a criminal, driving record check as well as Bureau of Elderly and Adult Services Must be able to provide eligibility to work in United States paperwork within 3 days of hire. Must have reliable transportation and be able to supply proof of car insurance in the amount of $100,000/$300,000 coverage. Access to operable motor vehicle and a valid NH driver’s license with acceptable record of safe driving practices. Willing to drive own automobile unlimited miles and in all types of weather conditions. Must have completed Direct Care Competencies with RN supervision before practicing independently. Ability to pay close attention to detail and follow verbal and written instructions. Ability to quickly adjust to change. Ability to swiftly assimilate and interpret new knowledge and/or regulations. Excellent interpersonal communication skills and organizational skills required. Computer skills strongly preferred.
