Responsible for identifying and prioritizing patient assessment needs based on clinical presentation and diagnosis. Initiates discharge planning upon admission and ensures practice aligns with regulatory and hospital policies.
Requires a minimum of three years of acute care clinical experience and completion of an accredited Registered Nursing program. A current California Registered Nurse license is required, with CCM or CPUR certifications preferred.
Minimum three (3) years of acute care clinical experience. Two (2) years’ experience clinical Case Management or Utilization Review preferred Completion of an accredited Registered Nursing program.
