The Discharge Planner identifies patient needs through assessment and planning to coordinate care between professional disciplines. They provide psychosocial support and counseling to patients and families to manage chronic or acute illnesses.
Candidates must hold a current Idaho RN or LPN license, or be a licensed BSW/MSW in Idaho. A current BLS certification is also required.
Description The Discharge Planner is responsible for identifying patient needs and priorities for care through assessment, planning, intervention, and evaluation. The Discharge Planner coordinates care between professional disciplines through documentation and oral communication as needed. The Discharge Planner will provide psychosocial support to patients and families so they can cope with chronic, acute, or terminal illness. Discharge Planner may advice family caregivers, counsel patients, and help plan for discharge needs.
