Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes. Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate. Conducts therapy practice within defined standards of care. Directs the assistance of support staff to achieve patients’ functional goals. Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures. Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community Physically demanding, high stress environment Full range of body motion including handling, lifting and transfer of patients Potentially work irregular hours including call hours if applicable 🎓 What is Required? Graduate of an APTA-accredited Physical Therapy program Current, unrestricted PT license in the state of practice Minimum one (1) year of experience in a healthcare setting Able and willing to travel throughout the coverage area Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. Must be able and willing to travel 50% Able to sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs. 👥 Who You Report To Branch Director, Clinical Manager, Clinical Supervisor, or designated leader. Requirements

Candidates must be a graduate of an APTA-accredited Physical Therapy program with a current, unrestricted PT license and at least one year of healthcare experience. A valid driver's license, dependable vehicle, and the ability to lift 50-100 lbs are required for travel-based home care.
