Documents patient care and monitors patient condition, communicating with care team, physician, patient, and family.
Assists patients with activities of daily living, including hygiene, elimination, nutrition, and ambulation.
Coordinates patient care across disciplines, including transfers, discharges, referrals, and support needs.
Provides patient and family education appropriate to age, culture, and circumstances.
Advocates for the physical and emotional well-being of patients.

What we’re looking for The right person who will provide direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.
